DLBCL Staging & Subtypes
DLBCL Staging

Providers typically use the Ann Arbor staging system. Staging lymphoma differs slightly from solid tumors because it is a blood cancer involving the lymphatic system. Staging is based on the number and location of affected lymph nodes. The stages include:

  • Stage I: Involvement of a single lymph node region
  • Stage II: Involvement of two or more lymph node regions on the same side of the diaphragm
  • Stage III: Involvement of lymph node regions on both sides of the diaphragm
  • Stage IV: Involvement of one or more organs (e.g., bone marrow, liver, kidney, or lungs) outside the lymphatic system

DLBCL Subtypes
  • ABC (Activated B-cell-like): Can be classified as high-risk or low-risk
  • Double-hit or triple-hit lymphoma: Also referred to as high-grade B-cell lymphoma

Essential Testing

A biopsy—typically involving removal of part or all of a lymph node—is required for a definitive diagnosis. The following tests are commonly performed on the biopsy sample: 

  • Immunohistochemistry staining: Identifies cancerous cells by staining surface or internal markers.
  • Flow cytometry: Classifies cells (B-cell or T-cell clones) and helps differentiate lymphoma types.
  • Biomarker Testing: Detects genetic mutations and classifies lymphoma into risk groups.
