Treatment approaches for DLBCL depend on factors such as disease stage and location, the patient’s overall health and age, laboratory and biomarker results, and possible side effects. Common treatment types include:
- R-CHOP: A five-drug regimen, given once every three weeks for up to six cycles. Includes rituximab, cyclophosphamide, doxorubicin hydrochloride, vincristine sulfate, and prednisone.
- Pola-R-CHP: Uses polatuzumab in lieu of vincristine, which is part of the R-CHOP regimen.
- This was tested against R-CHOP and showed improvement by about 5%-6% in terms of long-term remission.
- Dose-adjusted EPOCH: A chemotherapy regimen sometimes used before or alongside CAR T-cell therapy.
- CAR T-Cell Therapy: Targets CD19 or both CD19 and CD20. This therapy is often used in clinical trials for patients who relapse within the first 12 months of initial treatment.
- Bispecific Antibodies: Engineered to bind to two different antigens by combining regions from two different antibodies.
- Stem Cell Transplantation: Particularly beneficial for patients who relapse more than 12 months after their initial treatment.
